The 2026 Physician Fee Schedule introduced refinements to the Remote Patient Monitoring code family that change reimbursement math for most practices. Below is the practical summary — what changed, what didn't, and what to update in your billing workflow before the next cycle.
99453 — initial setup
Setup and patient education remains a one-time code, billable once per patient per episode of care. The 2026 national average is $19.27 (up from $18.48). Requirement is unchanged: at least 16 days of readings within the first 30-day period for the code to be reportable.
99454 — device + transmission, monthly
This is the per-month code that most practices live off. 2026 national average is $50.14 (up from $48.93). The 16-day readings requirement remains the binding constraint — if a patient does not transmit on 16 separate days in the 30-day period, the code is not reportable for that month.
Clarification this year: "separate days" means calendar days in the patient's local time zone, not 24-hour windows. This was a frequent denial reason in 2024 audits.
99457 / 99458 — management time
99457 (first 20 minutes/month of management time, by clinical staff under general supervision) increased to $51.20. 99458 (each additional 20 minutes) increased to $42.10. Both still require documentation of interactive communication with the patient — not just data review.
What to update
Two things in your workflow: (1) audit how you log the "separate days" denominator for 99454 to confirm time-zone correctness, and (2) update your fee schedule before January 1 — even a small per-encounter delta compounds across a 200+ patient panel.
